REGISTRATION STEPS

1. Visit connectcare3wellness.com
2. Click “Register”
3. Enter your First Name, Last Name, Date of Birth & Employer Code: 1159
4. Click “Lookup Account”
5. Begin your wellness journey!

Once registration is completed, you can access the Mobile App by downloading from either the Apple App Store for iOS or Google Play for Android.

MOBILE APP

1. Search for the "ConnectCare3" App in your respective app store and download.
2. On your desktop or mobile device, login to the ConnectCare3 platform (connectcare3wellness.com).
3. Once logged in go to the USER MENU and click on MOBILE ACCESS to generate a unique code.
4. A window will open, click on the NEW button. A unique access code will be generated in the Code field that you will be asked to enter for the app. This code will remain valid for 24 hours. A separate code is required for additional devices accessing the app.

Once registration is completed, you can access the Mobile App by downloading from either the Apple App Store for iOS or Google Play for Android.
